Instructions
1. Print forms. Print one Parent Intake per adult and print one Child Intake per child.
2. Hand-write your responses into the forms. (Please Note: You can download and fill-out the forms on your computer, however, completed forms CANNOT be saved to your computer. You will need to print hard-copies to save your responses.)
3. Scan/email or mail these forms before your phone intake with your facilitator.
4. Please bring all original forms to your first session if you emailed them.
